    Registration and consent form for the workshop
  • Dear Parent/Guardian,

    Thank you for being a part of Game Bloom: an educational workshop for adolescent girls and assigned females at birth on Performing in Puberty. The aim of this workshop is to improve the knowledge and skill on how to continue playing, performing and enjoying sport during the adolescent years. Additionally, we seek to delve into understanding the motivators and barriers influencing girls' participation in staying in sport, with the ultimate goal of fostering positive change in this domain.

    Workshop Details:
    - Date: Tuesday 19 th May
    - Time: 5:30 pm- 7:30pm (Arrive 5:15 pm)
    - Location: Clubrooms, Kelevdon
    - Duration: ~ 2 hours 
    - RSVP by 15th of May

    The workshop will cover various aspects including but not limited to:
    - Sports performance: Participants will learn strategies to enhance their athletic performance, including recovery, injury risk reduction, and injury management.
    - Understanding puberty: We will provide information on all the changes associated with puberty
    - Female Health: Menstrual cycle, pelvic floor, breast health, as well as addressing the common concerns and questions that adolescent girls may have.
    - Motivators and barriers: Through an interactive discussion, we aim to identify the factors that motivate girls to engage in sports as well as the obstacles they may encounter. Current research shows a high drop out rate of adolescence playing organised sport, particularly for girls.The future goal is to understand this topic better in order to improve the retention rate of girls playing sport.

    Our priority is your child's safety and well-being. As part of our commitment to ensuring a secure environment, we would like to assure you of the following:
    - Chrissy is an APA titled Sports & Exercise Physiotherapist who has worked as a physio for over 15 years.
    - All workshop facilitators have undergone thorough background checks and hold valid Working with vulnerable people checks.
    - Our team members are trained in safeguarding practices specific to sports settings, ensuring a safe and respectful learning environment for all participants.
    - We adhere to strict guidelines for privacy and confidentiality, respecting the sensitive nature of the topics discussed during the workshop.

    Additionally, please note the following:
    - Optional exercises: During some workshops, there are a small number of optional exercises. Participants are encouraged to participate at their own comfort level, and there is absolutely no pressure to join in if there are any injuries or concerns.
    - Photography consent: We may capture photographs or videos during the workshop for documentation and promotional purposes. Please indicate your preference regarding photography or video consent below.

    Please review and provide the information required below, and indicate your consent for your child to participate in this educational workshop.

    This workshop is directed at girls to attend with their peers and parents/guardians will receive their own summary on all the topics discussed during the workshop. However, if there is preference for a parent/guardian to also attend, then this is understandable and accepted. Please reach out if you have any questions or concerns.

    By hitting submit and electronically signing, I agree that I am the parent or legal guardian of the above named participant, and am over the age of 18.  I hereby give consent to Game Bloom workshop, powered by Unite Physio PTY LTD, to provide professional education and information on topics related to performance and puberty, and to allow for participation in any activities contracted under Game Bloom workshop unless I state directly to an employee of Game Bloom. I agree that the demonstration of exercises by participants is minimal and optional, and not recommended to anyone with a significant injury. I further agree that Game Bloom along with employees, officers and directors of this organisation shall not be liable for any injury that may occur as a result of my child’s participation in the program. I understand my child may be used in photos/videos solely for the social media accounts of Game Bloom unless I state otherwise within this form. I also understand that there are no refunds and or transfers of payment for the workshop.
