  • Please make sure you have submitted your HMIS APR (or if you are a VSP, an APR from a comparable database) via email to Chloe Chaplin (cchaplin@louhomeless.org). The period of performance must be April 1, 2025 - March 31, 2026. This will be used for scoring in the CFH-completed portion section of your rubric. 

  • New Projects: Threshold Criteria

    Threshold Criteria is required to be eligible for scoring and ranking. However, your responses do not hold a numerical score and will not impact your final score/rank.
  • Organizational eligibility: Applicant certifies that they are an eligible entity type as detailed in page 9 of the NOFO.
  • CoC Membership: Applicant is a member of the Louisville Continuum of Care. If no, Applicant must submit the Continuum of Care Membership Form prior to completing an application for funds.
  • HMIS Participation: Applicant is willing to participate in the HMIS system according to the expectations established in the Louisville CoC Data Quality Plan. If No, is the applicant a VSP who will utilize a comparable database?
  • Participation in Coordinated Entry system: Applicant is willing to participate in the Coordinated Entry system, as applicable. For questions on how Coordinated Entry may apply to your project, please contact Carey Addison at caddison@fhclouisville.org.
  • Participation in Point-in-time count: Applicant participated in the most recent Point-In-Time count conducted in Louisville or agrees to participate in future Point-in-time counts. .
  • Compliance with federal regulations: Applicant agrees to comply with all federal regulations* that govern the Continuum of Care Program, including those governing eligible program participants, and understands that it is the applicant responsibility to read, understand, and remain in compliance with said regulations. *Note: federal regulations can be found at https://rb.gy/lea8nn
