    If your complaint relates to a Safeguarding Concern, then please use the Report Safeguarding Concern form instead.

    If the JSA believes that there is a Safeguarding concern during the investigation of this complaint, then the JSA Safeguarding Officer will be informed.

    For more information about how the JSA will deal with Complaints, please review our Complaints and Appeals procedure.

    Once submitted, this form will be sent to the JSA Data Officers who will review your submission and ensure the relevant parties are then contacted and made aware of the complaint.
