Consent for Participation
I, the undersigned parent or legal guardian, give permission for my child listed above to participate in the Youth Disc Golf League and all related activities.
Acknowledgment of Risk
I acknowledge that participation in disc golf and outdoor recreational activities involves inherent risks, including but not limited to slips, trips, falls, being struck by discs, uneven terrain, and weather-related conditions. I understand these risks may result in injury.
Release of Liability
I release and discharge the Youth Disc Golf League, its organizers, volunteers, sponsors, and affiliated entities from any and all liability for injuries, damages, or losses arising from participation. I agree not to hold the league or its representatives responsible for any injury or accident that may occur.
Medical Authorization
I authorize league organizers and volunteers to obtain emergency medical care for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
Code of Conduct Agreement
I understand that my child is expected to follow all league rules and instructions, show respect to coaches, volunteers, and other participants, and practice good sportsmanship. Failure to follow these expectations may result in removal from league activities.
