**COMPLETE ONE REGISTRATION PER CHEERLEADER**

Cheerleaders must attend one Uniform fitting prior to the season (dates will be announced on our Facebook page). To ensure timely ordering of the uniform, the registration fee must be recieved no later than July 15, 2026

Equipment is the responsibility of the parents/guardians. For cheerleaders, this includes, white tennis shoes, white ankle socks, practice shorts (no jeans) and practice t-shirt.

All participants are required to participate in fundraising events/activities.

Power12welve requires that the registration packet include the player's birth certificate and proof of residence, which can include an utility bill and/or school verfication enrollment form be completed. Proof of residence and/or school verfication enrollment form will be collected at a later date.

Participants must live in Taft or attend school at Taft ISD to be eligible to play. Proof of school enrollment and/or residency is required.

Age Divisions - cut off date is Aug. 1st. Please note: Cheerleaders may cheer for siblings, family members, or friends if needed for transportation purposes. If a cheerleader is not needing to be assigned to a specific division, they will be placed in a division to cheer based on their age and/or skill level. Prior experience is not necessary to participate — our skilled coaches will provide all necessary instruction and support.

For safety reasons, athletic shoes are required for all practices. Absolutely no Crocs or non-athletic footwear will be allowed. Cheerleaders who do not wear proper shoes will not be permitted to participate in practice.

I hereby agree and declare that I am the legal parentguardian of the above-named participant and hereby consent to the participation in any activities and/or events hosted by Friends of the Taft City Parks. I understand that the activities/events may result in physical injury to the participant(s) listed above but nonetheless specifically request to be allowed to participate in those activities. I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the activities and I consent to the participation of the above-named participant(s) to these activities. I hereby release Friends of the Taft City Parks, its agents, directors, and affiliates, along with volunteers, Taft ISD, City of Taft and its successors, officers, agents, employers and institutions, from any and all claims, demands, and causes of action whatsoever in any way resulting from participation in any camp, league, game, practice and/or events. I also certify that my child has NO HEALTH OR PHYSICAL IMPAIRMENTS which will hamper his/her ability to perform as a player/cheerleader or cause his/her participation to be unsafe to themselves or other participants. If there are any health/physical problems Friends of the Taft City Parks should be aware of please indicate above. I personally and on behalf of the above-named participant, assume any and all risk that is in any way associated with, related to or occurs as a result of participation in football and/or cheerleading.

Friends of the Taft City Parks has my permission to use my or my child's photograph publicly to promote Friends of the Taft City Parks programs and the work they do in the community. I understand that the images may be used in print publications, online publications, presentations, websites and/or social media. I hereby agree not to make any claim for compensation for the use of my child(s) image. I hereby certify that I am the parent or guardian of the minor child named above and I have the legal authority to execute this release on behalf of the child (participant).

Parent's Code of Conduct

I have read the above Parent's Code of Conduct and I understand that if I disregard any of these rules, I can and will be asked to leave the premises with the chance that I may not be allowed back for the remainder of the season.

Insurance Clause: Friends of the Taft City Parks Youth Football League has Group Accidental Insurance Coverage for medical and hospital expenses, with a given deductible amount for each accident incurred. The League Insurance is considered as secondary coverage when there is any other valid and collectible coverage provided by the parent's/guardian's separate insurance. Maximum coverage is provided for any one accident with a given maximum dental coverage for sound, natural teeth. I understand that a claim for medical service, which arises out of injury, must be reported to a board member of the Friends of the Taft City Parks within twenty days of the date of injury. I understand that any registration fee or other sums paid does not contribute a direct premium payment for injury.

Medical Treatment Authorization Clause: In the event of injury or illness to my child, I hereby grant authority to a qualified physician to render such medical treatment as said physician deems necessary under the circumstances.

Pom poms are not included in the registration fee and are optional. New cheerleaders, or those who would like a set, may purchase pom poms for the season.

Returning cheerleaders who purchased pom poms for the 2025 season may use those pom poms for the 2026 season.

Zero Tolerance Policy Agreement

Friends of the Taft City Parks (FOTTCP) is committed to providing a positive, respectful, and supportive environment for all players, families, and volunteers.

We ask that all parents/guardians demonstrate good sportsmanship and respect toward the organization, board members, volunteers, coaches, players, the Power12 League and its board, and all participating teams and leagues — both in person and on social media.

If concerns arise, they should be addressed respectfully through the proper channels: first with the head coach, then with a FOTTCP board member and/or the President, rather than through public posts or negative commentary.

To protect the integrity of our program and those who support it, behavior involving disrespect, negativity, or harmful communication may result in action, including removal from facilities or impact on player participation.

By submitting this form, you acknowledge and agree to support these expectations and help us keep the focus on a positive experience for all children.
