    Thank you for your interest in booking a tattoo appointment! Please read through this form and fill out the questionnaire.
  • 🔹 Deposits & Booking
    • A $100 deposit is required to book and goes toward the final cost of your tattoo.
    • Major design changes after booking may require a new deposit and rescheduling.
    🔹 Cancellations & Rescheduling
    • 24 hours notice is required to reschedule your appointment for your deposit to transfer.
    • No-call/no-shows will forfeit their deposit.
    • An abundance of rescheduling may result in loss of deposit and refusal of future bookings.
    • If you are more than 30 minutes late, your appointment may need to be rescheduled.
    🔹 Respecting the Space
    • Please ask about bringing extra guests or children ahead of time.
    🔹 Payment
    • Remaining balance is due the day of your appointment.
    • Cash is preferred but I do accept card and Venmo payments
    🔹 Age & ID
    • You must be 18+ with valid ID to get tattooed.
    • Minors 16+ with parental consent and birth certificate in hand (no photos accepted). Please be aware there are size and placement restrictions that I have with tattooing minors. Parent must be present for the duration of the appointment.
    🔹 Health & Safety
    • Please let me know if you have any skin conditions, allergies, or are on medications that may affect your tattoo or healing.
    • If you are sick, it’s best to reschedule—please communicate with me.
    • If the roads are bad, don’t drive—please communicate with me.
    🔹 Touch-Ups
    • Touch-ups within 12 months are included. After that period, touch-ups will have a set fee.
    🔹 Communication
    • I understand that life happens and things can come up last minute. The most important thing is communication—if you reach out, I will always do my best to work with you.

  • I understand that this booking request form does not guarantee an appointment. I understand that I will be contacted for follow-up via email, and will be required to send a deposit within 24 hours of agreeing to an appointment time via Venmo, Paypal, or Apple Pay (unless otherwise discussed for cash drop offs). I acknowledge that until the deposit has been received, my appointment is not confirmed. If I need to cancel or reschedule my appointment, notice must be given 24 hours in advance for deposit to be transfered.
