2026-2027 Evaluation Registration

Please complete this form as accurately as possible to register your athlete for Galaxy Extreme Athletes 2026–2027 Cheer Season Evaluations. •Please submit one form per athlete •Athletes must be age 3 or older as of June 30, 2026 •We are a performance rec cheer organization and offer teams for all experience levels •Dance is a new program being considered for the 2026–2027 season based on interest Evaluation Fee - Cheer/Dance Evaluations: $60 per athlete; Returning Galaxy Athletes: $30; Dance Add-On Evaluation: $20; All evaluation fees are non-refundable.
Waiver Information

I give permission for my athlete, listed in this registration form, to participate in Galaxy Extreme Athletics evaluations for the 2026–2027 season.I understand that my athlete must attend one of the scheduled evaluation dates in order to be considered for placement on a team.I acknowledge that the evaluation fee is non-refundable, regardless of team placement or if I decide not to move forward with registration for the season.I understand that cheerleading and dance are physical activities that involve movement, stunting, tumbling, and other athletic skills that carry a risk of injury. While Galaxy Extreme Athletics takes precautions to promote safety through proper instruction, supervision, and equipment, injuries may still occur.By signing below, I acknowledge and accept these risks and agree that Galaxy Extreme Athletics, its directors, coaches, staff, and volunteers will not be held liable for injuries or accidents that may occur during evaluations, practices, events, or related activities.I also agree that my athlete will follow all rules, policies, and expectations established by Galaxy Extreme Athletics while participating in program activities.
