I hereby give permission to Limitless Sports Inc., to use photographs, voice recordings, or video taken of me and/or my player during the games and events associated with Limitless Sports Inc., to promote the league activities as determined in the sole discretion of Limitless Sports Inc. Cost for Registration, provided at the link below, is $100 for one athlete in a family, $150 for two athletes from the same family, and $200 for three athletes from the same family. In order to keep our programs accessible, scholarships are available due to the generosity of our donors. Does your family need scholarship support?

Limitless Yoga may take place outdoors on a field or court. While staff and volunteers are positioned throughout the area to support supervision and safety, there is still a potential risk of elopement (wandering or leaving the designated area). Please answer the following: 1. Does your child have a history of elopement/wandering?☐ Yes ☐ No If yes, please describe triggers, behaviors, and supports that help: By checking below, I acknowledge the potential risk of elopement and agree to share all relevant information to support my child’s safety. I understand that Limitless Sports cannot guarantee that all participants will remain within the designated area at all times. I also understand that I am ultimately responsible for communicating my child’s supervision needs and that I may be required to remain on-site or provide a 1:1 support person if deemed necessary for safety.
