Category Definitions:
1. Treatment decision: An issue arising from clinical judgment or lack of competency, such as the choice of interventions, medications, or procedures provided.
2. Refusal of care: A situation in which a patient, family member recommended medical evaluation, treatment or intervention, potentially delaying care and impacting the patient’s outcome.
3. Transfer issue: A situation where a facility, agency, or individual does not follow recommended interfacility transfer protocols, resulting in delays or barriers to timely access to appropriate care or specialized services
4. Variance from regional standards: Process differs from a Board-approved regional guideline where regional standards are defined.
5. Lack of regional standard: An issue identified where no regional guideline currently exists, leading to inconsistent care.
6. Operational logistics: Barriers related to system coordination, resource movement, or workflow operations.
7. Destination decision making: Concerns over the appropriateness of the patient’s receiving facility based on the patient’s preference, acuity or regionally available resources.
8. Human resource issue: Problems related to personnel availability that influences patient care and outcomes.
9. Material resource / equipment issue: Failures or shortages of equipment, technology, or supplies that impact patient care.
10. Communication failure: A breakdown in information sharing between EMS, hospitals, or other stakeholders that affects patient care.
11. Other: An issue that does not fit in the above categories but still represents a PI concern
