Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Choice Baptist Church during the 2026 VBS Program. In exchange for the acceptance of said child’s candidacy by Choice Baptist Church, I assume all risks and hazards incidental to the conduct of the activities and release, absolve, and hold harmless Choice Baptist Church, along with all its respective officers, agents, and representatives, from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from the selected 2026 VBS sessions.

In case of injury to said child, I hereby waive all claims against Choice Baptist Church, including all staff, teachers, affiliates, participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. I understand that there is a risk of injury inherent in all physical activities, including basketball. Some of these injuries may include, but are not limited to, fractures, paralysis, or death.

During Vacation Bible School (VBS), we may take photos and videos of activities to share the excitement and joy of the event on our church’s website and social media platforms. Please indicate whether you give permission for your child’s image to be used for these purposes. No names or identifying information will be shared. Your choice will be respected and will not affect your child’s participation in any way.
