    By signing below, I hereby authorize Missouri Valley Baptist Association to request the SafeChurch, or any entity chosen by the Association specifically for conducting this search, to release information regarding any record of charges or convictions contained in its files, or in any criminal file maintained on me, whether said file is a local, state, or national file, and including but not limited to accusations and convictions for crimes committed against minors, to the fullest extent permitted by state and federal law. I do release said police/sheriff’s department and other entities from all liability that may result from any such disclosure made in response to this request. By signing below, I also confirm that all information is true and that I have disclosed any criminal occurrences’ since the last records check was ran.

  • MEDICATIONS WILL BE CHECKED IN TO THE CAMP NURSE UPON ARRIVAL AT GRAND OAKS. THIS INCLUDES ANY OVER THE COUNTER MEDICATIONS SUCH AS TYLENOL.

  • This information will be accessible to the Camp Directors and Camp Nurse. This form will be kept on file in the MVBA office.

  • MEDICAL RELEASE: I (we) have provided complete and accurae inforation about his camper/staff member and understand that, in the event medical treatment is required, every effort will be made to contact me (us) or the person named above. However, if I (we) cannot be reached, I (we) give permission to the staff or sponsor to secure the medical services deemed necessary to provide for this camper's/staff member's wellbing. I (we) also understand that the insurance provided by Grand Oaks Baptist Association, Inc. is a limited supplmental policy covering only injury or accidents during the event at Grand Oaks. Even then it will be used only to supplment the family insurance.

    • Bible
    • Towel (For Bathing & swimming)
    • Pillow
    • Sleeping bag, or blankets and sheets (Twin Size Beds)
    • Notebook
    • pen or pencil
    • Soap, comb, hairbrush, toothbrush, deodorant, etc. (Any items necessary for good hygiene)
    • Flashlight
    • Reusable Waterbottle
    • Camera (this is optional)
    • Insect repellant/sunscreen
    • Casual Clothing (No midriffs or halter tops)
    • Underwear & Socks
    • Sweatshirt or light jacket
    • Shoes (tennis shoes for recreation)
    • Raincoat/Umbrella Pajamas (worn in their cabin only)
    • Swimwear (1-piece or Tankinies with no midriff visible for girls)* Swimsuit cover up or t-shirt.

    Please note: Clothing should be comfortable and in keeping with the fact that this is a Christian Camping experience. No T-shirts with offensive lettering or pictures will be allowed. This includes, but is not limited to, shirts that advertise beer or tobacco. Please mark all items clearly. We are not responsible for lost or misplaced items.

    Dress clothes are not necessary, but we do ask that enough clothes be sent SO that the camper has a clean outfit for chapel worship each night.

    Electronics devices are not allowed. Please do not bring them to camp. We are trying to limit distractions that would prevent concentrating our attention on our relationship with Christ.

    NO CELL PHONES WILL BE ALLOWED ON CAMPUS, THEY WILL BE TAKEN AWAY AND GIVEN BACK ON FRIDAY IF YOU BRING THEM. (FOR STUDENTS. Staff need to limit use and use with discretion)

    Bringing snacks from home is not allowed (food is not allowed in the cabins, because it attracts bugs and other "critters" If a student has a medical condition that requires snacks, please note on the medical form.

    *The camp committee requests that the girls wear one-piece bathing suits or Tankini that covers the torso AT ALL TIMES. If a two-piece suit is brought, it must be worn with a dark T-shirt or cover-up. All campers are required to cover swimsuits with T-shirts and/or towels when not in the swimming pool area.

  • Children's Camp: July 20-24 (Staff Check-in 10-11am, ends at 9:45 am on Friday) TRAINING: July 19, 4 pm at Wakenda Baptist Church

  • Should be Empty:
