Minor Photo Release

I hereby confer on Gridley Public Library District the absolute and irrevocable right and permission with respect to the photographs that he/she/it has taken of my minor child in which he/she may be included with others:

a) To use, re-use, publish and re-publish the same in whole or in part, separately or in conjunction with other photographs, in any medium now or hereafter known, and for any purpose whatsoever, including (but not by way of limitation) illustration, promotion, advertising and trade, and;

b) To use my name or my child’s name in connection therewith if he/she so decides.

I hereby release and discharge Gridley Public Library District from all and any claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy. I also give Gridley Public Library District permission to post my child’s name with his/her reading accomplishments.

I have read the foregoing and fully understand the contents hereof. I represent that I am the [parent/guardian] of the above named minor. For value received, I hereby consent to the foregoing on his/her behalf.

*All photo/information release forms will be kept on file at the library for one calendar year.*

Program & Activity Sign Ups

To help us better prepare for participation we are going to have sign-ups for many of our programs this year. If your child is interested in participating in any of the following activities, please sign them up. If plans change, let us know so we can extend the opportunity to someone else!

Please indicate the total number of children in your family that will participate in each activity.  We do not need individual names for each activity for registration, but we do need to know how many to plan for!

**For the READ with Grace & Tucci activities, there is only 1 participant slot for each time for each pup.  If you want multiple times, you will need to select the different times.  You can bring younger kids and have older kids read to them as well as Grace or Tucci, but will need to stay within the 15 minute time frame.
