  • DECLARATION: I consider myself to be physically fit and capable of full participation and agree to notify the Club of any changes to the medical information provided.  In the event of injury I give permission for the team coach/members appointed by the Club to obtain medical treatment on my behalf. I acknowledge that CRICKET can be a dangerous sport and I participate in TRAINING (including Warm Up) AND MATCHES at my own risk.  I realise that Maghera Cricket Club and Management Committee Members / Committee Members/ Officers are not liable/ responsible for any injuries caused while training and playing matches.*
  • MEDIA: I would like to be included in media for club promotional purposes - including, but not limited to photography, video, audio or print. I understand that from time to time the club will give the rights to third parties for editorial and broadcasting purposes (this applies to all media, including social media).*
  • GDPR: I agree to my personal data (name, postal address, age, gender, religious background and telephone number) to be held by Maghera Cricket Club (MCC) on a secure database and shared with the North West Cricket Union for administration and publication purposes.  A copy of the MCC Privacy Statement can be accessed by contacting the MCC Secretary on magheracricket@yahoo.com*
