If selected to receive this scholarship, I agree to provide proof of my final GPA and any additional documentation required for award disbursement. I also grant Phi Nu Kappa Sorority, Inc. permission to use my name, likeness, and photograph in publications, news releases, online communications, and other materials related to the promotion and presentation of this scholarship.
