Medical Release
In the event of an illness or injury, I authorize church staff and volunteers to obtain necessary medical attention for my child. I understand that reasonable efforts will be made to contact me immediately in the event of an emergency. If I cannot be reached, I give permission for my child to receive appropriate medical treatment, including first aid and emergency services, as deemed necessary. I agree to assume responsibility for any medical expenses incurred.

I understand that photographs and/or video in which my child appears in may be used by the church to create a highlight video of the week to be shared during VBS, following Sunday Church Service, and church’s website.
