Materials needed: a) this application; b) uploaded PROOF OF INSURANCE: Evidence of liability insurance naming NAACP San Luis Obispo County Branch as additionally insured, with a minimum of $1,000,000 comprehensive general liability insurance; c) IF you are selling items and have a SLO City business license, a copy of the license; d) payment.
