This form is for submitting a tattoo appointment request! Once I have all your info for a custom or pre-drawn tattoo, I'll reply via email with a quote and link for booking yourself into my calendar with a $75 deposit. Response times are usually within one week, if you don't hear back by then, please check your spam folder! Please note, my current tattoo station is not ADA accessible, so please be sure to note in the comments section so I can make arrangements to tattoo you out of an accessible station.
