Alcohol, Smoking and Substance Involvement Screening Test
About This Screening
This is a brief, confidential self-assessment to help you understand your current pattern of use of tobacco, alcohol, and other substances. It is called the ASSIST-Lite, a tool developed by the World Health Organization (WHO) and used in healthcare settings worldwide.
You can fill this yourself, or a family member can fill it on behalf of someone they are concerned about.
How to fill this form
Answer each question based on your experience in the past 3 months only
For each substance, answer "Yes" or "No" — there are no right or wrong answers
Additional questions appear automatically only if you have used that substance
It takes about 3 to 5 minutes to complete
Is this confidential?
Yes. Your responses are used only to generate a personalised result for you. The Centre for Addiction Medicine, NIMHANS may review de-identified responses to improve services. No information is shared with employers, insurers, or any third party.
What happens after you submit?
You will receive an email with your results, including a risk level for each substance (Low, Moderate, or High). The result also tells you what steps, if any, are recommended based on your score.
If any substance shows a High risk level, we encourage you to speak with a local doctor government hospital or medical college and take advice. You are welcome to visit us in person to NIMHANS Hospital OPD on Monday/Thursday/Saturday. The details are available at the NIMHANS website nimhans.ac.in .
This screening is not a diagnosis. It is a starting point to help you or your family make informed decisions about seeking support. If you are in distress right now, please call Tele-Manas 14416 (toll free).
