Faith Formation Registration
The Importance of the Mass
The Eucharist is the source and summit of our Catholic faith around
which we gather as a family in the Church. Holy Mass is the highest and
best form of catechesis. Participation in the Mass is essential to Faith
Formation, as it is where we encounter Christ most fully
in Word and Sacrament. Regular Mass attendance is expected for all
families participating in Faith Formation.
If your family is not yet ready to commit to weekly Mass attendance, it
may be better to wait before enrolling so that your child can fully benefit
from this formation experience.

Middle & High School Students Only.(Currently Discipleship Teams meet on Wednesday evenings from 6-7.)
Class times and days are subject to change.
Final session details will be sent prior to the start of classes.
Tuition/Fees

$100 for the first child + $25 for each additional child
The Retreat and Sacrament Supply fee for those receiving First Communion or
Confirmation in the spring is $75 per child.
Emergency Medical Release & Liability Waiver
In the event of an emergency, I authorize St. Bernadette Catholic Church to obtain medical treatment for my child if I cannot be reached. I understand that reasonable efforts will be made to contact me.

I release and hold harmless St. Bernadette Catholic Church, its staff, and volunteers from any liability in connection with participation in Faith Formation activities.

Safe Environment Program
St. Bernadette Catholic Church follows the Protecting God's Children program. All children will receive age-appropriate safe environment training twice per year.
