I consent to the processing of my personal data (Data) by the Fundacja Instytut Innowacji, Al.Jerozolimskie 65/79, Warsaw 00-697 ( Fundacja), for purposes related to the organisation by the Fundacja of the event referred to in this registration activity (Event) and my attendance at the Event in accordance with Regulation(EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC(General Data Protection Regulation). The data may be my name and surname, email address, age, and information about my professional background. I acknowledge that the Fundacjais the data controller of the Data it will obtain in connection with my participation in the Event. The consent I hereby give for the processing of Data is voluntary and free. I have been informed that I have the right to withdraw my consent to the processing of the Data in its entirety or to the extent selected at any time by any means that can be identified and captured. Should the Fundacja have any doubts as to the scope of the declaration of withdrawal of consent to the processing of Data, the Fundacja will adopt an interpretation that extends such declaration in order to best protect my interests and the Data, to inspect the Data, to request the correction, verification, portability and the right to request their deletion without undue delay, which, however, does not affect the fact and lawfulness of the processing of the Data prior to such my decision, to lodge a complaint with a supervisory authority: General Inspector for the Protection of Personal Data(from 25.05.2018: President of the Office for the Protection of Personal Data),2 Stawki Street, 00-193 Warsaw, when I consider that the processing of the Data is unlawful.
