Medical Release
I certify that I am in good health and able to participate in camp activities. I give my consent for the Director or a properly appointed staff member of Nebraska NxtGen Ministries to secure and authorize emergency medical treatment on my behalf, including transportation, hospitalization, treatment, surgery, and/or medications, as deemed necessary by licensed medical professionals.
I understand that every reasonable effort will be made to contact my emergency contact in the event of an emergency. However, if contact cannot be made, I authorize such treatment as deemed necessary for my well-being. I accept full responsibility for any medical expenses incurred.
Assumption of Risk & Liability Release
I understand that participation in camp activities involves inherent risks, including recreational activities, travel, and physical exertion. I voluntarily assume all risks associated with my participation.
I agree to release, waive, and hold harmless Nebraska NxtGen Ministries, the Nebraska Ministry Network, The Crossing Retreat Center, and their respective directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur during my participation, except in cases of gross negligence.
Leader Conduct & Agreement
I understand that serving at Nebraska NxtGen Ministries Camp is both a privilege and a responsibility. I agree to:
-Conduct myself in a manner consistent with biblical standards and Christian character
-Follow all camp policies, procedures, and expectations outlined in the Camp Staff Manual
-Support and submit to camp leadership and direction
-Create a safe, respectful, and spiritually healthy environment for students
-Refrain from any inappropriate, harmful, or unscriptural behavior
-I understand that failure to comply with these expectations may result in dismissal from camp at my own expense.
Pastoral Acknowledgment
I give permission for Nebraska NxtGen Ministries to contact my pastor or church leadership, if needed, regarding my character and suitability for serving with students. I waive any right to review references provided on my behalf.
Media Release
I grant permission for Nebraska NxtGen Ministries and the Nebraska Ministry Network to use photographs, video, and audio recordings of me for promotional, educational, and ministry purposes in any medium, without compensation.
Dress Policy Agreement
I understand and agree to follow the camp dress policy:
Only appropriate and modest clothing is permitted. Clothing displaying indecent, suggestive, or profane writing, images, or slogans is not allowed. Shirts must not expose the midriff or sides of the torso (for both males and females).
Ladies: Bra straps and bralettes must be covered. Leggings must be worn with tops that extend to mid-thigh. Shorts and skirts must be at least mid-thigh length.
Camp staff reserve the right to require a leader to change attire if deemed inappropriate.
Background Check Acknowledgment (If not already completed)
I understand that serving at camp may require a current background check in accordance with Nebraska Ministry Network policies. I agree to comply with any required screening process.
