This form grants permission for my student, a rising 9th grade football player at Trickum Middle School, to use Gwinnett County Public Schools (GCPS) after-school transportation from Trickum Middle School to Parkview High School for football-related activities and acknowledges parent/guardian responsibility for pickup.

Transportation Details
I give permission for my student to ride the GCPS after-school bus from Trickum Middle School
to Parkview High School for participation in football workouts, conditioning, practices, or
related team activities.
I understand and agree to the following:
1. My student will be transported from Trickum Middle School to Parkview High School
via GCPS transportation for football-related activities only.
2. I am fully responsible for arranging prompt pickup of my student from Parkview High
School following completion of all activities.
3. I understand that supervision of my student after arrival and following the conclusion of
football activities is limited to standard school and athletic program procedures.
4. I acknowledge that failure to pick up my student on time may result in disciplinary action
or loss of transportation and/or participation privileges.
5. I release and hold harmless Trickum Middle School, Parkview High School, their staff,
coaches, and Gwinnett County Public Schools from liability related to transportation and
post-activity supervision.
