Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by Astro Aftercare Camp.

Media Disclosure Astro Skate may periodically photograph students participating during camp. These photographs may be used in a variety of ways; Public relations and marketing purposes: images printed on brochures, posters, and newsletters, or on Astro Skate Website, Facebook Page, and/or other art, education, or news related publications. Please indicate whether you will allow publication of photographs taken of your child/children during camp. Names will not be included in photographs or posted on social media. We appreciate your support and would love the chance to share pictures of all of the fun being had at our Summer Camp. You can also follow us on Facebook to capture all of the pictures yourself!

CONSENT, LIABILITY WAIVER & TRANSPORTATION AGREEMENT

I hereby give my consent for my child to participate in the Astro Skate of Ormond Beach Camp Program located at:

I understand this is a for-profit camp recreational program designed to provide opportunities for exercise, athletic participation, and various educational and recreational activities.

3. ASSUMPTION OF RISK & LIABILITY WAIVER

I acknowledge that, even with the use of reasonable care by Astro Skate of Ormond Beach, participation in program activities involves inherent risks, which may result in serious injury or, in rare cases, death.

These risks include, but are not limited to:
☐ Cuts, bruises, and scrapes
☐ Sprains and broken bones
☐ Other physical injuries

By signing below, I voluntarily assume all risks associated with my child’s participation.

To the fullest extent permitted by law, I hereby release, waive, and hold harmless Astro Skate of Ormond Beach, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to my child’s participation in the program.

4. TRANSPORTATION AUTHORIZATION

I authorize Astro Skate of Ormond Beach to transport my child to and camp related activities using Astro Skate Aftercare transportation vehicles.

I understand that transportation may occur following regular camp hours and may include trips to parks or other approved locations as part of program activities.

I acknowledge and accept the risks associated with transportation.

5. ACKNOWLEDGMENT & SIGNATURE

I have read and fully understand this Consent, Liability Waiver, and Transportation Agreement. I acknowledge that I am signing this document voluntarily and that it is legally binding.

BULLYING POLICY

Astro Skate of Ormond Beach maintains a zero-tolerance policy for bullying, harassment, or aggressive behavier of any kind.

All participants are epxected to treat others with respect and alwasy follow program rules.

Zero tolerance for bullying or harassment
All incidents must be reported immediately to staff
Physical altercations may result in immediate disciplinary action, including possible suspension or expulsion from the program

I understand the terms of the Astro Aftercare Bullying Policy.

Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
