Milwaukee - Youth Entrepreneur Showcase Application
Thank you for your interest in participating in the Youth Entrepreneur Showcase. This is a hands on opportunity for youth to create, sell, and manage their own small business during a live event.

This is not just a table to sell items. This is a learning experience. Participants are expected to prepare, show effort, and actively run their own table.

This application should be completed by the youth participant as much as possible. Parents or guardians may provide guidance when needed, but responses should reflect the youth’s own ideas and effort. The parent or guardian section should be completed by the parent or guardian.

This is an application-based program. Submission does not guarantee acceptance. Applications will be reviewed, and selected participants will be notified.

Youth should answer the following questions in their own words.
What is the name of your business? (if you have one)
What do you plan to sell? Describe your product in detail. (If you are planning on offering baked goods or drinks, please be descriptive and confirm ingredients are shelf-stable and would pass non-hazardous food guidelines)
Do you make or plan to create these items yourself? If not, explain your role in preparing or customizing the product
Why did you choose this business idea?
Briefly describe how you plan to pay for your materials or supplies.

What are you planning to do to get ready before the event?
What will you do if you feel nervous or unsure when talking to customers?
Section 5: Program Policy Acknowledgement
Please review the full Youth Entrepreneur Showcase Program Policies before submitting this application. The policies outline youth responsibilities, parent and guardian roles, participant cost expectations, product guidelines, and event requirements.

By submitting this application, the youth participant and parent or guardian confirm that they have fully read, understand, and agree to follow all program policies. Failure to follow these policies may result in removal from the event.

Section 6: Hold Harmless and Liability Waiver (Parent or Guardian Required)
By signing below, I acknowledge that my child is voluntarily participating in the Youth Entrepreneur Showcase.

I acknowledge and accept all risks associated with my child’s participation, behavior, and any products they bring or sell.

I release and hold harmless the Hmong Wisconsin Chamber of Commerce, event organizers, partners, staff, volunteers, and the event venue from any and all liability, claims, damages, losses, or injuries that may arise from participation in this event.

I understand that the program organizers are not responsible for lost, stolen, or damaged items, including money or products.

I understand that my child is responsible for managing their own sales and materials during the event.

In the event of a medical emergency, I authorize event staff to seek medical care for my child if I cannot be reached.
Section 7: Photo and Media Release (Parent or Guardian Required)
I grant permission to the Hmong Wisconsin Chamber of Commerce and its partners to photograph, record, and use images or video of my child during the event for promotional, educational, and marketing purposes.

This may include use in print materials, social media, websites, and other communications.

I understand that no compensation will be provided for use of these images or recordings.

I understand I may opt my child out of photography or video by notifying organizers in writing prior to the event.
