Baptism is a joyful moment in the life of the Church, and our parish is honored to walk with your family as you bring your child into the faith. This form is the first step in preparing for the sacrament. If this is your first child being baptized, we will also need to schedule a baptismal preparation class with you. After you submit this form, I will personally reach out to you to help schedule your child’s baptism date.

Godparent Information
Requirements for Parents and Godparents
- At least one parent or legal guardian must be a practicing Catholic.
- At least one godparent must be a confirmed, practicing Catholic, age 16 or older. Godparents should be receiving the sacraments frequently and if married, must be married in accordance with the Catholic Church.
- Only one Godparent is required; if there are two, they must be male and female.
- If a godparent cannot be present, a Catholic proxy may be designated
