1. COMPLIANCE WITH PROGRAM RULES AND POLICIES

I agree to ensure my child follows all GEWD Athletics guidelines, rules, policies, and codes of conduct as set by the coaching staff and organization leadership.

2. VOLUNTEER AND CHAPERONE PARTICIPATION

I agree to serve as a chaperone, driver, or volunteer when called upon by GEWD Athletics for team travel, tournaments, or program-related activities.

3. PHOTO, VIDEO, AND MEDIA RELEASE

I grant GEWD Athletics permission to photograph and video record my child for use in marketing, promotion, social media, fundraising materials, and community outreach.

4. DISCIPLINARY ACTION

I understand that repeated violations of team rules, unsportsmanlike conduct, or disrespectful behavior may result in my child's suspension or expulsion from the program at the discretion of GEWD Athletics leadership.

5. LEGAL CUSTODY CONFIRMATION

I confirm that I am the legal custodial parent or court-appointed guardian of the registered athlete and have the authority to consent to these terms on their behalf.

6. TRANSPORTATION CONSENT

I authorize my child to travel in vehicles arranged or approved by GEWD Athletics for practices, games, tournaments, and team events, provided the vehicle is operated by a licensed adult driver.

7. MEDICAL AUTHORIZATION

In the event of a medical emergency, I authorize GEWD Athletics staff and coaches to administer emergency first aid or arrange medical transport if I cannot be reached.

8. ASSUMPTION OF RISK AND LIABILITY RELEASE

I acknowledge that youth tackle football involves inherent physical risks and I release GEWD Athletics, its officers, coaches, and volunteers from any and all liability arising from injury or accident during program-related activities.
