8:00 - 9:00 - Session with Jen Lepak

9:00 - 11:00 - Movie and Popcorn

12:00 - Lights Out

 

Saturday, May 23rd

9:00am - Breakfast

10:00 - 12:00 - Session with Jen

12:00 - Lunch

1:00 - 3:00 - Team Games

3:00 - 5:00 - Free Time

5:00 - Supper

6:00 - 8:00 - Session with Jen

8:00 - 10:00 - Bonfire

10:00 - Pickup and Head Home

All physical activities have risks.  I agree to allow my child to participate in this event knowing there are physical risks and hazards inherent in any activity.  These may include but are not limited to damages to property, injuries, and diseases.
This event is not identified as a higher risk activity. (Higher risk activities include active sports and overnight activities.)
I voluntarily agree to allow my child to participate in this event.  By voluntarily allowing participation, I agree that I am exposing my child to inherent risks and hazards.  I agree to accept all risks and hazards and be responsible for any injury or other loss which may occur during the participation of my child.
I understand safety precautions will be taken at all times by Grace Community Church and its agents during the events and activities. I authorize any treatment by hospital and/or physician deemed necessary for the subject of the release in case of an emergency.

I (Student), am signing this form agreeing to participate in all events for the weekend, and being present at all times. I agree to not leave the group without permission and to sit through services, engaging in what's happening. I agree that after 2 warnings, I will be sent home.
