    🎓 Recipients of Academic Awards & Study Tour Scholarships

    Who’s invited:
    ✨ Students, advisors, families, alumni, and community supporters are all welcome to attend!

    • Distinguished Silver & Gold Clubs: Up to 3 officers + advisor
    • Neighbor Island Clubs: PAAC will cover flights & Oʻahu transport for 3 officers + 1 advisor; other members are welcome if self-funded (PAAC will provide meals & ground transport)
    • Award recipients on Oʻahu: May bring up to 2 family members (They each must register)
    • Community members and alumni: All guests must register

  • Medical - Emergency Consent

  • In the event my child becomes ill or incapacitated, I understand that every effort will be made to contact the parent(s)/guardian(s) or emergency contacts. If no one can be reached, I authorize PAAC staff, its board, agents, and contractors to take whatever action they deem necessary regarding my child’s health and safety, including securing medical treatment and, if needed, arranging for their return home—at my expense.

    I further release PAAC, its board of governors, directors, employees, agents, contractors (domestic and international), the Hawaiʻi State Department of Education, my child’s school, and any persons accompanying the group from liability related to such actions or decisions. I understand PAAC is not responsible for the quality, timeliness, or cost of any medical care received.

  • Release, Policies, and Indemnity Agreement

  • This agreement is made between the Pacific & Asian Affairs Council (“PAAC”), the undersigned student (“Student”), and the Student’s parent(s)/guardian(s).

    By participating in a PAAC-sponsored event (including but not limited to summits, workshops, and showcases), the Student and their parent(s)/guardian(s) understand and agree to the following:

    Participation & Conduct

    1. Program Oversight:
      All arrangements related to transportation (when provided), accommodations, meals, and the event schedule are under the direction of PAAC staff.
    2. Travel to/from Events:
      When traveling independently, the Student and parent(s)/guardian(s) are responsible for transportation to and from the home airport or designated meeting point. If PAAC arranges transportation, those logistics will be communicated in advance.
    3. Transportation Use:
      PAAC staff may use public or private transportation for instructional or extracurricular activities as needed and will notify families accordingly.
    4. Behavioral Expectations:
      1. The use of tobacco, alcohol, or illegal drugs is strictly prohibited and may result in immediate dismissal from the program at the Student’s and family’s expense.
      2. Students are expected to follow all rules and codes of conduct established by PAAC staff, including dress and behavior guidelines.
      3. PAAC reserves the right to dismiss any Student whose conduct or health status makes continued participation inadvisable. Any costs associated with an early return home are the responsibility of the Student and their family. Refunds will not be issued in such cases.
    5. Health & Well-being:
      Students must be in good physical and mental health and disclose any relevant conditions or special needs prior to the program.
    6. Property & Liability
      1. Personal Property:
        PAAC is not responsible for loss or theft of Student’s personal items.
      2. Safety & Responsibility:
        Students are expected to act with caution and care to prevent harm to themselves or others.
      3. Property Damage:
        Students are responsible for any damage they cause and agree not to hold PAAC, its affiliates, or partners liable. They agree to cover associated costs.
    7. Evaluation & Feedback: Students agree to complete follow-up paperwork, including evaluations, some of which may be used anonymously in PAAC publications.
    8. Legal Provisions: Governing Law: This agreement is governed by the laws of the State of Hawai‘i. If any portion is deemed invalid, the rest shall remain in effect.

    Release, Hold Harmless, and Indemnity
    In consideration of the benefits derived from participation in this program, the Student and their parent(s)/guardian(s) voluntarily release and discharge PAAC, its board of governors, directors, officers, employees, agents, contractors, volunteers, chaperones, the Hawaiʻi Department of Education, the Student’s school, and all associated personnel from any and all claims, liabilities, causes of action, and costs—including those related to property damage, illness, injury, or other risks—arising from or related to participation in this event.

    The Student and parent(s)/guardian(s) further agree to indemnify, defend, and hold harmless all aforementioned parties from any such claims or liabilities.

  • Student Publication/Audio/Video Release Form

  • The undersigned Student and the Student’s parent(s)/guardian(s) hereby grant permission to the Pacific & Asian Affairs Council (PAAC) to create, use, and publish the Student’s name, voice, likeness, images of Student work, video and audio recordings, and photographs for the following purposes:

    Use on PAAC’s websites, social media platforms, or other digital/electronic media;
    Inclusion in printed materials such as newsletters, brochures, and promotional materials.

    TERMS OF CONSENT

    1. No Compensation:
      I understand that neither the Student nor the parent(s)/guardian(s) will receive compensation, financial or otherwise, for the use of these media—either for initial publication or for any future transmission or playback.
    2. Privacy Acknowledgment:
      I acknowledge that granting this permission involves a potential loss of privacy. I hereby release PAAC from any liability connected to the creation, use, or publication of these media.
    3. Duration of Consent:
      I understand that this permission is granted for the life of the media. I waive any rights or interest in the media, including the right to inspect or approve its final form.
    4. Right to Withdraw:
      I understand that I may withdraw this permission at any time by submitting a written request to PAAC. Withdrawal of consent will not affect media that has already been created or published prior to the date the written request is received.
  • Student Behavior Code

  • While traveling and participating in PAAC activities, behavior consistent with PAAC’s goals and image is expected. All students are expected to follow the school rules outlined in Chapter 19 concerning student conduct and general behavior by the Hawaiʻi State Department of Education. Failure to do so will result in your being sent home, potentially at your own expense.

    Community Guests, Volunteers and Family
  • Release, Policies, and Indemnity Agreement

  • This agreement is made between the Pacific & Asian Affairs Council (hereinafter referred to as “PAAC”) and the undersigned Participant, and any guest(s) accompanying the Participant, who may be a guest, family member, volunteer, or community member (hereinafter referred to as “Participant”).

    TERMS OF PARTICIPATION

    By participating in a PAAC-sponsored event (such as a student showcase, summit, workshop, or similar activity), the Participant understands and agrees to the following:

    1. Program Oversight:
      All aspects of the event, including the schedule, choice of venues, activities, and meals, are under the direction of PAAC staff.
    2. Personal Property:
      PAAC is not responsible for the loss, damage, or theft of any personal belongings of the Participant.
    3. Safety and Conduct:
      The Participant is expected to exercise reasonable care, caution, and common sense to avoid situations that could result in personal injury or harm to others.
    4. Damage to Property:
      The Participant accepts responsibility for any damage they may cause to property during the event and agrees to cover all associated costs. The Participant further agrees not to hold PAAC, its board of governors, directors, staff, agents, contractors, affiliated schools, or accompanying individuals liable for such damage.
    5. Event Feedback:
      The Participant may be asked to complete a post-event evaluation or feedback form. Excerpts from this evaluation may be used in future PAAC publications or promotional materials, unless anonymity is specifically requested.
    6. Governing Law:
      This agreement shall be governed and interpreted in accordance with the laws of the State of Hawai‘i. If any part of this agreement is found to be invalid, the remainder shall remain in full force and effect.

    RELEASE, HOLD HARMLESS and INDEMNITY

    In consideration of the benefits derived from participation in the event, the Participant voluntarily releases and discharges PAAC, its board of governors, directors, officers, employees, agents, representatives, volunteers, chaperones, the Hawai‘i Department of Education, and affiliated schools from any and all claims, liabilities, causes of action, costs, and expenses—including but not limited to those related to personal injury, illness, property damage, or other risks—which may arise from or relate to participation in this event.

    Furthermore, the Participant agrees to indemnify, defend, and hold harmless all of the above-mentioned entities and individuals from any and all such claims or liabilities arising from their participation.

  • Participant(s) Publication/Audio/Video Release Form

  • The undersigned Participant, who may be a guest, family member, volunteer, or community member (hereinafter referred to as “Participant”), and any guest(s) accompanying the Participant, hereby grant permission to the Pacific & Asian Affairs Council (PAAC) to create and use photographic, video, audio, or other media recordings of the Participant for the following purposes:

    • Publication on PAAC websites, social media platforms, or other digital/electronic communications;
    • Use in printed publications, presentations, or promotional materials.

    TERMS OF CONSENT

    1. No Compensation:
      I understand that PAAC will not provide any form of financial or other compensation for the creation or use of these media, whether now or in the future.
    2. Privacy Considerations:
      I acknowledge that media use may carry a risk of reduced personal privacy. I voluntarily accept this risk and release PAAC from any liability related to the use or publication of such media.
    3. Duration of Consent:
      This permission is granted for the life of the media. I waive any rights or claims of ownership, royalty, or control over the use of the media once this form is signed.
    4. Withdrawal of Consent:
      I understand that I may revoke this consent at any time by submitting a written request to PAAC. However, revocation will not apply retroactively to media already created, published, or distributed prior to receipt of the written request.
