Youth Participation Agreement & Medical Release Form

I hereby Give the named participant permission to attend and participate in the activities of Cedar Ford Baptist Church youth ministry. I hear by wave all claims against Cedar Ford Baptist Church, it’s employees, and volunteers, in case of an accident or injury that may arise as a result of participation at activities, including Sunday, school events, youth group events on or off the Cedar Ford, Baptist Church campus, either sponsored and part, or in whole, by Cedar Ford Baptist Church. If necessary, the employees and volunteers of Cedar Ford Baptist Church will treat, to the best of their abilities.

Cedar Ford, Baptist Church youth Ministry events will be supervised at all times by the employees and volunteers. If at any time, the child intentionally leaves the supervised areas, or if the child does not respond to discipline from the adults, the guardian will be notified, and the child will be asked to be picked up.

The youth, participation agreement and medical release form gives permission to Cedar Ford Baptist Church to treat the child in the event of a medical emergency. It releases Cedar Ford Baptist Church of any liability or monetary responsibility, incurred during medical treatment.
