Program Lottery Form
For the upcoming school year, Highland will be utilizing the lottery system to fill our available program seats. Children can be signed up for programs between May 4th - May 10th, 2026. Families will be notified by May 22nd, 2026 as to whether or not they received their requested seats or if they've been placed on the waitlist.
Please complete this form in it's entirety. If your child receives a seat, this completed form will serve as your contract.
No need to rush, take your time and make your program selections from the available programs below. Programs are grouped by grade levels. Please do not make any payments with your submission. You will receive separate correspondence regarding whether or not your child was accepted into programs or waitlisted. That correspondence will also let you know when deposits/payments will be due for the programs you selected.
If you have more than one child, you will have to complete a separate program lottery form for each child.

Adolescent Program Before Care
If you qualify for Wisconsin Shares and your child is accepted to programs, you will need to do the following:
1. Contact MES at 888-947-6583 to add Highland Community School as your childcare provider. Our Provider #9000580989 - Location #001 - EBT Facility #E268468. Your childcare authorization has to be updated by August 30th in order for your child to start programs on their first day of school. If your authorization is not updated by the 30th, you will have to pay the full cost of the first months program fees in order to keep your child's seat in programs.
Note: An adjustment will be made to your account for students staggered in after the first day of school.
*There are a limited number of part-time attendance spots in the After Care & K3 Extended Care programs. Part-time spots are limited to 1) Mon, Wed & Fri or 2) Tues & Thurs only. Fees will be prorated based on days selected. All terms apply to part-time attendance as well as full-time. Additional Billing Information: Invoices for the entire month will be posted on the 1st of the month and a text message will be sent. This is a prepay program. Program payments are due by the 5th of each month (i.e. May fees will be billed at the end of April & payment is due by May 5th.
Pay in advance for the year and receive a 4% discount or pay per semester and receive a 2% discount.
