• Athlete Photo Registration Form

    Please fill out the form below. Fee is $ 20 per athlete. Each athlete will be allowed 5-10 minutes for photos unless in group photos (amount of time is up to photographers discretion). Each athlete will receive 5-10 edited images (depending on poses, groupings, etc).

  • Important Photo Instructions
    • Athletes must be in full uniform, including shoes.
    • If you have spike shoes, please remove the spikes for the photos.
      (Photos with shoes hanging on shoulder can have them, any full body shots with shoes on must have spikes removed to not damage the backdrop).
    • We have one gold batton to use for a prop anything else you want to have in your photos you must bring yourself.

    I, the undersigned parent/guardian or athlete, give permission for photographs of the athlete to be taken during the date listed above. I understand that these photos may be used for promotional purposes, including by not limited to social media, websites, printed materials, and other marketing related to BWMN Sports.I acknowledge that no compensation will be provided for the use of these images.By signing below, I confirm that I am the athlete or parent/legal guardian of the athlete listed above and that I agree to the terms of this photo release.
