Hello, This form is for submitting an internal claim. With this form you can submit for the following needs: Warrenty claims, Non Warrenty claims, annual maintenance requests and claims within the four month workmanship guarentee after installation.

In general, most all requests submitted within the four month workmanship guarentee after installation are fully covered out of our goal to provide customer satisfaction and value for our homeowners. Warrenty Claims are subject to trip charges. Non Warrenty Claims are subject to Service fees and trip charges. Annual Maintenance for any waterproofing system is $150 and any structural system Annual Maintenance is $250 with no trip charges within an hour radius of Springfield MO.

Requests for Warrenty claims and Non Warrenty claims are subject to the terms set forth in the service agreement and limited warrenty statment provided in the initial statment of services.
