This form must be completed by the parent or guardian who originally completed the Fall 2026 SASA soccer registration. Refund requests must be submitted using the same name and email address associated with the registrant’s GotSport profile. Refund requests will not be processed without submission of this form or if the request cannot be matched to the original registration.
Submission of a refund request does not guarantee a refund. All refund requests are reviewed by the SASA Board, who will determine whether a refund is approved and the amount, if any, to be returned. Full refunds are not guaranteed due to administrative, insurance, and league expenses. Approved refunds may be partial and are issued at the discretion of the SASA Board. Refund eligibility may be affected by the stage of the season, including whether registration has closed, teams have been formed, or rosters have been distributed to coaches. No refunds will be issued once the Fall 2026 season has begun.
Review Process
Once submitted, the request will be reviewed by the SASA Board at a board meeting. A SASA or Board representative may contact you if additional information is needed. You will be notified via email once a decision has been made regarding your request.
