• Flu Exposure Assessment

    If you have been exposed to someone with the flu within the last 48 hours and do NOT yet have symptoms, we can now prescribe medication for you to help prevent the flu if you qualify (i.e. were exposed less than 48 hours ago, are at least 5 years of age, do not have end stage renal disease/not on dialysis, haven't taken antiviral medication for flu within the past 4 weeks and are considered to be in a high-risk flu category). *The cost of the medication is dependent on which medication is prescribed and your copay.

  • Are you experiencing any of the following symptoms*
  • Per the state protocol, we are unable to provide treatment if you are experiencing any signs or symptoms of the flu. Please refer to our test and treatment service.

  • Have you received the influenza vaccine FluMist® Nasal Spray in the last 2 weeks?*
  • Have you ever had an allergic reaction to medicine used to treat the flu? For example, did you have trouble breathing, a rash, or swelling after taking flu medicine like oseltamivir (Tamiflu®)?*
  • Have you ever had any side effects after taking flu medicine in the past? (like nausea, vomiting, diarrhea, abdominal pain, dizziness, difficulty breathing, wheezing) *YesNo*
  • Are you on dialysis, or has your provider ever told you that you have kidney failure?*
  • Per the state protocol, we are unable to provide treatment if you have End Stage Renal Disease (ESRD) on hemodialysis or peritoneal dialysis. Please contact your primary care physician.

  • Have you taken medicine for the flu in the last 4 weeks? (like Tamiflu or another antiviral)?*
  • Per the state protocol, we are unable to provide treatment if you have taken an antiviral treatment for influenza in the past 4 weeks. Please contact your primary care physician.

  • The following questions are to assess if you are in a high-risk flu category.

  • Are you pregnant or breastfeeding?*
  • Are you currently receiving treatment for cancer, such as chemotherapy or radiation (this includes treatment for blood cancers like leukemia, lymphoma or multiple myeloma)?*
  • Have you ever had an organ or stem cell transplant and are you currently taking medications to prevent your body from rejecting it?*
  • Have you been taking medications that weaken the immune system, like steroids (such as prednisone) for more than 2 weeks?*
  • Has a provider ever told you that you have a weakened immune system due to a condition you were born with such as DiGeorge syndrome, Wiskott-Aldrich syndrome or Bruton Disease?*
  • Do you have sickle cell anemia?*
  • Have you ever been diagnosed with HIV or AIDS?*
  • Do you have any other condition that significantly affects your immune system?*
  • Do you use oxygen at home or need extra oxygen to help you breathe?
  • Are you 19 years of age or younger and taking aspirin every day or on a regular basis for a health condition?
  • Has your provider ever told you that you have kidney problems, or kidney disease, but that you don’t need dialysis?*
  • Has your provider ever told you that you have breathing problems, such as asthma, COPD, or cystic fibrosis?*
  • Has your provider ever told you that you have heart problems such as heart disease, heart failure, blocked arteries or problems with blood flow to your heart?*
  • Do you have diabetes?*
  • Are you of American Indian or Alaska Native descent?*
