I Agree that the West Cook YMCA shall not be responsible for any personal injuries or losses sustained by me while on any YMCA premises or as a result of YMCA sponsored activities.

I further agree to indemnify and hold harmless the West Cook YMCA from any claims or demands arising out of any such injuries or losses.

If I am asked, I also agree to leave the facility due to inappropriate behavior as defined by the YMCA staff. A refund will not be given if a guest is asked to leave.

I agree that I will follow all rules and procedures as outlined in the West Cook YMCA member handbook.

If this is a rental, I agree to follow the terms outlined in the rental agreement even if I am a participant and did not sign the rental agreement. I agree to follow up with the person organizing the rental to know and understand those terms.
