I, ______________________________________, the parent/guardian of the registrant, ______________________________________, a minor, hereby agree that both the registrant and I will abide by the rules and regulations of the Mondo International Football Academy.

I certify that my child is physically fit and capable of participating in all activities conducted by the academy.

Having considered the benefits that my child will gain and recognizing the possibility of physical injury associated with sports participation, and in consideration of the academy accepting the registrant into its soccer programs and activities, I hereby release, discharge, and indemnify Mondo International Football Academy, its coaches, staff, and associated personnel, including the owners of the fields and facilities used, from any and all claims arising from the registrant’s participation in the program.

I also authorize Mondo International Football Academy to use photographs and/or videos taken of my child during the program for promotional and official materials.

Furthermore, I acknowledge the significant investment made by Mondo International Football Academy in my child’s football and academic development. I agree that my child will not participate in or register with another club or academy without obtaining prior written clearance from Mondo International Football Academy.
