    PLEASE NOTE:

    Completion of the following application in its entirety is required for participation in the Delta Sigma Theta Sorority, Inc., Collin County Alumnae Chapter youth programs.

    Please allow approximately 15–25 minutes to complete the application and required forms. You may need signatures from multiple parties, including:

    ·        the applicant,

    ·        the parent/guardian, and

    ·        if applicable, the alternate teen driver’s parent/guardian

    ·        personal statement to be completed by the applicant

    As part of the application process, applicants will be required to complete a personal statement reflecting on their interest in and ability to commit to program participation.

    We strongly encourage families to begin the application only when they have enough uninterrupted time to carefully review instructions, complete all required sections, and upload any necessary documents.

    If multiple youth from the same family wish to participate, a separate application and all required forms must be submitted for each child individually.

  • Youth Application Overview

    Collin County Alumnae Chapter

    Since its founding in 1913, Delta Sigma Theta Sorority, Inc. has clearly distinguished itself as a public service organization that boldly confronts the challenges of African Americans and, hence, all Americans. Over the years, a wide range of programs addressing education, health, international development, and strengthening of the African American family have evolved.  In realizing its mission, Delta Sigma Theta provides an extensive array of public service initiatives through its Five Point Programmatic Thrust, Delta Academy, Delta G.E.M.S, EMBODI, and the Collin County Alumnae Chapter's Annual Cotillion.

     Delta Academy:

    The Dr. Betty Shabazz Delta Academy is a unique program designed for young ladies ages 11-14 years (grades 6-8). The goals of the Academy are to enhance or spark interest in math, science, technology, and/or careers where minority women are scarcely represented. The Dr. Betty Shabazz Delta Academy’s aim is to shape well-rounded young women by focusing on African American history, literacy, character development, healthy choices, and service learning.

    Eligibility and Requirements:

    • Young ladies, 11 to 14 years of age (grades 6-8), who have potential for success but limited opportunities.
    • Young ladies who are interested in developing themselves and sharpening various skills (i.e., literacy, math, and science).
    • Young ladies identified by schools, churches, youth groups, or concerned individuals as candidates.
    • Young ladies interested in service, education, and technology.
    • Young ladies who enjoy learning new things.
    • Young ladies who express interest in math, science, technology or other non-traditional careers.

    Delta G.E.M.S:

    The Delta G.E.M.S. program addresses the needs of young African American young ladies in grades 9 through 12 (Ages 14 – 18) with a framework for the participants to discover and understand their individual brilliance by embracing the aims of the G.E.M.S. theme, Growing and Empowering Myself Successfully. The Delta G.E.M.S. program includes monthly workshops and outreach activities to engage young ladies in experiences for personal development in the areas of leadership, self-awareness, community service, and college & career preparation.

    Eligibility and Requirements:

    • Young ladies, 14 to 18 years of age (grades 9-12), who have potential and need guidance, support, and skills to achieve success.
    • Young ladies interested in developing leadership skills.
    • Young ladies who are interested in actively pursuing future college and/or career options.
    • Young ladies who could benefit from encouragement and support in pursuit of higher learning.

    EMBODI:

    EMBODI (Empowering Males to Build Opportunities for Developing Independence) is a national program sponsored by Delta Sigma Theta Sorority, Inc. The purpose of the program is to focus on improving the plight of the African American male. EMBODI is designed for boys who are in 8th - 12th grades.

    Eligibility and Requirements:

    • Young men who are no younger than 13 years old and no older than 18 years old as of their most recent birthday (grades 8-12).
    • Young men who have potential and need guidance, support, and skills to achieve success.
    • Young men who are interested in actively pursuing future college and/or career options.
    • Young men who could benefit from encouragement and support in pursuit of higher learning.

    Cotillion:

    This nine-month youth development program is designed to educate, enlighten, and empower high school juniors and seniors, while enhancing their personal growth as these young ladies transition into adulthood.  Through a series of life skills workshops and cultural events, the selected teens will learn etiquette and social graces, prepare for college, and participate in service projects.  Upon completion of this program, CCAC will present our Debutantes to society. This is a fee-based program, and acceptance is contingent upon meeting all requirements.

    Eligibility and Requirements:

    • Applications will be considered until the capacity is met.
    • Applicants must be a 2026-2027 high school senior for Debutante status, or a 2026-2027 high school junior for Lady in Waiting status.
    • Applicants must be a female Junior or Senior between the ages of 16 and 18.
    • Applicants must have a cumulative GPA of 2.5 or better, based on a 4.0 grading scale (clearly identified on and verified by the official transcript).
    • Applicants must demonstrate interest and involvement in public service and community-related activities.
    • Applicants must demonstrate goals, aspirations, and plans to pursue an education beyond high school.
    • Applicants must have evidence of good moral character, which includes but is not limited to:
      • No record of poor discipline at school or in the community.
      • No criminal, misdemeanor, or court record of any kind.
      • Must not be a parent.
      • If you are currently pregnant, or become pregnant during the Cotillion Season, you will become ineligible and not allowed to continue participating in Cotillion activities and the Cotillion presentation.
    • Applicants must not have previous or current employment in places of entertainment, i.e., bars, clubs, etc.
    • Applicants must be able to commit to attending all necessary rehearsals and activities that occur throughout the Cotillion Season
    • If selected, debutante applicants understand that an Escort is required to participate, and applicants are responsible for securing an Escort to present them at the cotillion ball.
    • If selected to participate in the fee-based Cotillion program, applicants and their families must agree to adhere to all program guidelines. These include, but are not limited to: timely payment of all required fees, compliance with the youth code of conduct, adherence to the dress code, and demonstration of respectful behavior.

    The completed application must include:

    • Delta Academy, Delta G.E.M.S, EMBODI, and Cotillion: Application must be completed and signed.
    • Cotillion: A copy of an official school transcript with a minimum 2.5 GPA based on a 4.0 grading scale. The GPA must be "clearly identified" on the transcript. Transcript copies are to be emailed to ccac.cotillion@gmail.com.
    • Cotillion: Letter of Recommendation (can not be from a family member) must be signed and emailed directly to ccac.cotillion@gmail.com from the individual supplying the recommendation.

  • Confidentiality Policy

  • It is the policy of Collin County Alumnae Chapter of Delta Sigma Theta Sorority, Incorporated ("DST") to protect the confidentiality of its youth participants and their families.  Except as provided below, Chapter will only share information about participants and their families with other Delta chapter members and Delta employees assigned to assist with youth initiative programs, on a "need to know basis."

    To carry out the mission of its youth programs (Delta Academy, Delta GEMS, EMBODI and the Cotillion) and to better serve the needs of the youth participants, the Collin County Alumnae Chapter must collect certain personal information about youth participants and their families, including, but not limited to, the following "Confidential Information":

    • Name, address, and age of participant
    • School participant attends
    • Names and addresses of parents or guardian
    • Medications and physical conditions/limitations
    • Any distinguishing marks or characteristics (such as disfigurement or physical limitations)

    Limits of Confidentiality: Confidential information may be shared with individuals or organizations as specified below under the following conditions, and provided that the party who seeks any disclosure agrees in writing to maintain the confidentiality of the disclosed information as specified in this Confidentiality Policy:

    • Delta Officers and Members of the Board have access to any participant's files only upon directive by the National President.  Any directive shall identify the person(s) authorized to review such records; the specific purpose for such review, and the period of time during which access shall be granted.  Such Officers or Members of the Board granted access shall be required to comply with this Confidentiality Policy and may use the information only for purposes specified in the National President's directive.
    • Information may only be provided to law enforcement officials or the courts pursuant to a valid and enforceable subpoena or court order.
    • Information may be provided to Delta's legal counsel in the event of litigation or potential litigation involving Delta and/or the Program participants or any aspect of the Program.
    • Members of the Chapter and volunteers who observe or suspect child abuse are 'mandatory reporters' and, as such, must disclose suspected abuse to the proper authorities, and in making such reports, may disclose "Confidential Information."

    Safekeeping of Confidential Records: The President of the Collin County Alumnae Chapter or her designee shall be the custodian of confidential records.  It is her responsibility to supervise the management of Confidential Information in order to ensure safekeeping, accuracy, accountability, and compliance with this Confidentiality Policy.

    Requests for Confidential Information by Other Agencies: Any request from other organizations or persons for Confidential Information shall be honored only if the request is accompanied by written authorization from the parents or guardians of the youth participant expressly permitting the release of the requested information.

    Violations of Confidentiality: Known violations of this Confidentiality Policy (by volunteers or youth participants) shall be reported to the chapter president or her designee. A violation of this Confidentiality Policy shall result in disciplinary action up to and including suspension or termination from the Program, as appropriate.

    No Liability: There shall be no liability to Delta Sigma Theta Sorority, Incorporated, the Collin County Alumnae Chapter, or any volunteer or youth participant for disclosing information that is required to be disclosed by a court, an administrative body of competent jurisdiction, a governmental agency, or by operation of law.

    IMPORTANT BEFORE YOU BEGIN:

    Please pause and carefully review the previously provided youth program descriptions to ensure you select the correct, age-appropriate program for your child.

    Please note that the Cotillion Program is only open to high school juniors and seniors and requires a participation fee.

    Cotillion applications are not considered complete until both an official transcript and a letter of recommendation have been submitted. Families are encouraged to submit these items by the August 1, 2026, application deadline, if possible. However, if additional time is needed, the application itself must still be submitted by August 1, 2026, and transcripts and recommendation letters may be submitted afterward by email to ccac.cotillion@gmail.com.

    Before submitting, please read each application question thoroughly and complete all sections carefully. Applications completed too quickly often contain incorrect or incomplete information, which may delay processing or affect eligibility.

    We strongly encourage families to take their time and review all entries for accuracy before submission.

    All applicants are required to complete the following personal statement independently. This response should reflect the applicant’s own thoughts, goals, and commitment to participation in the program.

    CCAC youth programs are designed to provide meaningful personal growth, leadership development, sisterhood/brotherhood, community engagement, and educational enrichment through monthly activities and events held throughout an eight-month program year. Many activities may take place on Saturdays and/or Sundays and require regular attendance and participation.

    In a well-developed response of at least 250 words and no more than 500 words, please explain:

    · why you are interested in participating in a CCAC youth program,

    · how you believe the program will help you grow personally, socially, academically, or as a leader, and

    · how you plan to balance participation in the program alongside your school responsibilities, home life, and other extracurricular activities.

    Applicants should thoughtfully consider the time commitment required for participation before submitting their application.

  • Applicants for the Cotillion Program are required to have a recommendation letter submitted on their behalf to complete the application process.  The recommendation letter must include the information below, and must be emailed directly to ccac.cotillion@gmail.com, by the *individual supplying the recommendation (letters received directly from the applicant will not be accepted):
