  • CUSTOMER RELEASE FOR HIGH PRESSURE CYLINDER AIR FILLS

    In consideration for being permitted to receive an air fill of compressed air in a scuba cylinder or other approved cylinder hereinafter "cylinder" from Cruise Fish Dive Inc., I hereby expressly and contractually promise and agree as follows:

    1. Your cylinder should not be used if it is, or is suspected to be, leaking. Your cylinder should be inspected once every twelve months, or more frequently in certain cases. The visual inspector can help you decide how frequently your cylinder should be inspected. If it is time, or, you suspect or have any concern about the general condition of your scuba cylinder, a visual inspection by an experienced visual inspector is needed.

    2. The cylinder will never be used, by me or anyone for scuba diving.

    3. The cylinder will not be used with a scuba regulator for breathing off of.

    I understand and am aware of the inherit dangers associated with the use of compressed air/gasses and specifically assume those risks. I expressly release Cruise Fish Dive, its employees, owners, owner's heirs, agents and assigns, for any and all liability arising as a consequence of any and all personal injury, property damage or wrongful death caused by or associated with the use of the cylinder, on behalf of myself, my heirs and anyone else I expose to the dangers associated with the compressed air/gasses.

    I release Cruise Fish Dive and its employees, owners, owner's heirs, agents and assigns, for any faults or accidents that could happen with any of the cylinders I had filled at Cruise Fish Dive.

    I understand the purpose of signing this release is to give up all my rights to sue the above named dive shop, its employees, owners, owner's heirs, agents and assigns, or insurers and that the document constitutes a legally binding contract. This document is to be interpreted as broadly as possible, rather than narrowly construed, in the event of a legal dispute.

    I agree if there is a legal dispute I will be responsible for all legal fees for both parties and possible damages to the above named dive shop, Cruise Fish Dive. 

    I have read and understand the foregoing in its entirety and agree to the terms and conditions herein above set forth on behalf of myself, my estate, my heirs, and my personal representative(s). I have had an opportunity to ask questions regarding this document prior to signing the same.

  •  CUSTOMER RELEASE FOR CYLINDER INSPECTION FORM

    Thank you for considering us for your high pressure scuba cylinder visual safety inspection. The regular visual safety inspection of your scuba cylinder is part of its routine care and maintenance. With proper care, maintenance, regular visual inspections and hydrostatic retesting (retesting is done every five years), your cylinder will provide you a long, safe and satisfactory service.

    Visual safety inspections need to be done on your scuba cylinder to assure its continued safe service. The frequency of these inspections depends upon the kind of use, care and maintenance your cylinder gets. The more often it is used, the worse its care, maintenance and handling, the more frequently it should be inspected.

    Your cylinder should also be inspected before further use whenever it is known to have been exposed to temperatures over 350°F, been in an accident, known or suspected to be leaking, dropped, fell, was struck, has obvious corrosion since the last visual inspection, has a gouge, dent, scrape, cut, dig or, in any way, has been damaged since the last visual inspection, or was stored with water, material or matter inside the cylinder.

    Your cylinder should not be used if it is, or is suspected to be, leaking. Your cylinder should be inspected once every twelve months, or more frequently in certain cases. The visual inspector can help you decide how frequently your cylinder should be inspected. If it is time, or, you suspect or have any concern about the general condition of your scuba cylinder, a visual inspection by an experienced visual inspector is needed.

    The visual inspection is a safety check on your cylinder and the inspector performing it wants to make sure that your scuba cylinder is safe for continued service. If, during the inspection, a cylinder condition exists which would make your scuba cylinder unfit for continued service, it will be necessary to condemn and to render your cylinder unusable. The condemned cylinder, valve, and accessories will be returned to you with an explanation of the condition found. By signing below, you agree to this process which may render your scuba cylinder unusable in the future.

    I agree, as a condition of having this visual inspection, that the cylinder may be disabled and/or rendered unusable if it does not meet the accepted standards of quality established by the manufacturer for continued service. I will receive a written report of any and all conditions which caused the scuba cylinder to fail the inspection and to be condemned.  Also, I will have the condemned cylinder, accessories, components, and valve returned to me. 

    I have also been made aware that Cruise Fish Dive recommends that my cylinder valve be serviced every year when my cylinder is Visually Inspected and the Burst Disk should be replaced every 5 years during the cylinder's Hydro-Static Test.  I understand that if I refuse service to my cylinder valves, then I release Cruise Fish Dive and their employees for any faults or accidents that could happen with any of the cylinders I had filled at CFD, on any of my upcoming dives using tanks in question.

    I understand and agree that neither Cruise Fish Dive or its staff, "the Released Parties", filling the cylinder(s) with compressed air, may be held liable or responsible in any way for any injury, death or other damages to me, my family, estate, heirs or assigns that may occur as a result of my use of any cylinders filled by Cruise Fish Dive or its staff.

    I further release, exempt and hold harmless Cruise Fish Dive and its staff of any claim or lawsuit by me, my family, estate, heirs or assigns, arising out of my injury of death from the result of using the cylinder filled by Cruise Fish Dive or its staff.

    I further state that I am of lawful age and legally competent to sign this liability release, or that I have acquired the written consent of my parent or guardian. I understand the terms herein are contractual and not a mere recital, and that I have signed this Agreement of my own free act and with the knowledge that I hereby agree to waive my legal rights. I further agree that if any provision of this Agreement is found to be unenforceable or invalid, that provision shall be severed from this Agreement. The remainder of this Agreement will then be construed as though the unenforceable provision had never been contained herein.

    I understand and agree that I am not only giving up my right to sue the Released Parties but also any rights my heirs, assigns, or beneficiaries may have to sue the Released Parties resulting from my death. I further represent I have the authority to do so and that my heirs, assigns, or beneficiaries will be estopped from claiming otherwise because of my representations to the Released Parties.

                VIP Service. VIP Service puts you at the front of the line. If you are having an annual visual inspection & valve cleaning, we provide a 24hr turn around. Some restrictions apply. Call for details and if you need a valve rebuild, make sure parts are available if selecting VIP Service. Does not apply to Hydro Testing.



                Tank Fills from 0psi (Pick Tank Size). Standard Air Fills are filled with compressed air. To get a full fill, please drop your tanks off or leave with us overnight. We follow all PSI/PCI, DOT & OSHA specifications for handling cylinders. All Cruise Fish Dive Staff have completed the Cylinder Hazmat FSO Compliance Training via PSI/PCI. You may see we do things differently than other shops, and charge a little more, but this is to insure you and our safety as well as to protect your equipment. Never should a tank be filled in a water bath, over filled to let it cool to the tanks working pressure, or be filled fast. The speed of a fill is not to exceed 300-600 psi/min for 21% at 70 degrees. For a standard 80 cu. aluminum tank, it should take around 8-10 minutes minimum for a proper fill at 70 degrees, and longer if the air temp is warmer.                 Complete Valve Service. Complete rebuild for any standard K or DIN Tank Valve. Must be done every year at time of visual inspection. Valve service will be performed on all tanks when a Visual Inspection is performed. We replace all seals, seats and completely clean the valve per manufactures specifications. Performed by a PSI/PCI Certified Technician.

                Genesis Tank Valve Rebuild. Complete rebuild for Nitrox Genesis Tank Valve with stem replacement. Must be done every year at time of visual inspection. Valve service will be performed on all tanks when a Visual Inspection is performed. We replace all seals, seats and completely clean the valve per manufactures specifications. Performed by a PSI/PCI Certified Technician.

                Burst Disc. Burst disc must be replaced at time of every hydrostatic inspection.
