Please supply one image of your work to appear in the brochure.  Ideally, this wants to be square or standard portrait/landscape proportions; other shapes may not fit within the format.  Please provide an image that is just of your work, without background distractions or frame.  Images should be JPG, in RGB colour system, 300 dpi, approx. 1Mb (1000 Kb). All images need to be clean, sharp and the right way up.

We prefer to receive the image with the form.  However, if you have difficulty, please email it to contactdttv@gmail.com, and ensure it is clear whose entry the image is for.

You can save the form to continue it later, but please be aware that until you have ticked the confirmation button below and then pressed Submit, your entry will not have been received. You will receive confirmation of receipt of your application with a copy of the submission.
