Photo and Video Image Release Agreement

PLEASE READ THE FOLLOWING INFORMATION CAREFULLY

By agreeing below, I, the undersigned, hereby grant consent and unqualified authorization to GetSportsFocus, including its owners, officers, employees, successors, assignees, and anyone acting under their authority or permission, to create, copyright, and use photographs and video recordings of me. These materials, referred to as “Photographs” and “Video” herein, may include reproductions of my likeness, voice, name, and/or image.

I understand and agree that these materials may be published, used, and distributed at any time and as often as desired, in any format or medium, for any commercial, public, or private purposes. These purposes include, but are not limited to, advertising, promotion, and other public displays. I further waive any and all rights of inspection or approval over the final products or their use.

By agreeing to this statement, I confirm that I am at least eighteen years of age and acknowledge the above terms without reservation.

Waiver and Release of Liability

PLEASE READ THE FOLLOWING INFORMATION CAREFULLY

As a coach participating in the GetSportsFocus (GSF) athletic programs, events, and activities, I, the undersigned, acknowledge, appreciate, and agree to the following:

Assumption of Responsibility: I recognize and accept the risks associated with participation in this program, including the potential for injury or harm. I acknowledge that while rules, equipment, and personal discipline can mitigate these risks, they cannot eliminate them entirely. I knowingly and freely assume all such risks, both known and unknown, including those arising from the negligence of GetSportsFocus, its staff, or others.

Professional Conduct:

I agree to carry out my role as a coach in a professional, ethical, and respectful manner.
I will not engage in any conflict, dispute, or inappropriate conduct with other coaches, players, spectators, staff, or anyone involved in the event.
I will maintain a positive environment, upholding the values of sportsmanship and teamwork, and act in the best interest of the athletes and the event.
Acknowledgment of Consequences: I understand that failure to adhere to the above expectations, including unprofessional or inappropriate behavior, may result in disciplinary action, including but not limited to:

Immediate removal from the event.
Suspension or permanent exclusion from future GSF events.
Possible legal action requiring consultation with legal representation.
Acceptance of Risk: I willingly accept responsibility for my actions and their consequences, and I agree to comply with all stated terms and conditions for participation. Should I encounter any significant safety hazard or issue during my presence or participation, I will immediately notify an event official.
Release of Liability: I hereby release, indemnify, and hold harmless GetSportsFocus, its owners, officers, employees, agents, and affiliates, from any and all claims, injuries, disabilities, or damages to person or property arising from or related to my participation in these programs, whether caused by negligence or otherwise, to the fullest extent permitted by law.
Insurance and Protection for All Parties:

GetSportsFocus is fully insured to ensure the safety and protection of all parties involved in the event.
This waiver and release form is part of the process to protect everyone—coaches, players, staff, and the organization itself—in the event of any unexpected incidents.
We fully trust and value our coaches, and this formality is in place to ensure that we are all covered and can focus on creating a positive and successful event experience.
By signing below, I confirm that I have read and understood this Waiver and Release of Liability. I accept these terms freely and voluntarily, without coercion or inducement, and I commit to upholding the values and standards of the GSF All-Star program.
