USTA League Team Confirmation & Captain's Agreement
A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the information listed below and sign the form. Captains must sign the form for the team to be included in the match schedule.
Completed team confirmations must be submitted by June 19th, 2026 to be included in the league season.
