- For FACEMs: I am in Good Standing with ACEM, currently compliant with all Fellowship renewal requirements, including satisfactory participation in the CPD Program and completion of the latest Renewal of Fellowship Declaration.
- For Trainees or Other Categories: I confirm that I am enrolled in the FACEM Training Program.
- I have no outstanding financial obligations to the College, including the payment of the annual subscription fee.
- I hold current medical registration with no suspensions, conditions, restrictions, or undertakings imposed by any regulatory authority that would limit my ability to practise as a specialist emergency medicine physician in Australia or Aotearoa New Zealand.
