Online Pharmacy Purchase Consent

Magley Animal Hospital wants to ensure our patients receive the safest and most effective pharmaceutical products available. However, we do understand the desire to find products at a reduced cost. Several out of state, online pharmacies are able to sell products at a slightly lower cost than is possible for us, but we strive to stock and sell them at a competitive price. Please read the following information so that you can make an informed decision before purchasing products from other sources:

1. All guarantees by the product manufacturer are null and void if their products are purchased through any source other than a veterinarian. If there is a failure in effectiveness, then you are solely responsible for the cost of treatment.
2. Our products come directly from the manufacturer and are guaranteed.
3. We are not aware of where these products are being distributed from, such as being brought from overseas and not in original packaging, so therefore we cannot be sure the product is as stated.

If you prefer to purchase your products from another source, then we will approve the prescription according to the following guidelines:

1. An exam and diagnostics have been done within a necessary time frame.
2. A current heartworm test has been done with a negative result.
3. If there is a problem with the product or shipment, you are to handle it with the online pharmacy directly.

I understand the concerns of buying from an online source, and will not hold Magley Animal Hospital liable for any issues that arise from these purchased products.
