  • International Pet Travel Waiver

  • Traveling with pets internationally can be a complicated, time-consuming, and stressful process involving multiple organizations and government entities. Animal Hospital of Fort Lauderdale (AHOFL)'s part is to examine the pet(s) to ensure they are healthy for travel, to make sure all necessary medical items are completed, and to submit the appropriate paperwork to the USDA for endorsement. After that, AHOFL has little to no control over the health certificate documents. The USDA queues the health certificate according to date of travel and endorses it only 1-2 days before it is needed (they endorse only Monday through Friday). They then ship the documents overnight (this can be from anywhere in the U.S.) via UPS or FedEx to the address of your choice. While every effort is made to get the documents to you on time, there is potential for unforeseen shipping delays, which may result in a situation where you do not receive your documents in time for travel. Traveling internationally with your pet(s) should not be undertaken lightly and should only occur if there are no other alternatives.

I understand that Animal Hospital of Fort Lauderdale has no control over weather conditions, carrier delays, or other unforeseen circumstances that may impact delivery times.   • PET TRAVEL RESPONSIBILITY & LIABILITY WAIVER AGREEMENT

    This Pet Travel Responsibility & Liability Waiver Agreement ("Agreement") is entered into by and between Animal Hospital of Fort Lauderdale ("Hospital") and the undersigned client ("Client") on the date indicated below.

    1. Client Responsibility for Travel Requirements

    The Client acknowledges and agrees that they are solely responsible for reviewing, understanding, and complying with all current pet travel requirements, including but not limited to those established by the United States Department of Agriculture (USDA) and the Centers for Disease Control and Prevention (CDC), as well as any requirements of the destination country and any countries of transit. The Client understands that such requirements may change at any time and may vary by country, and agrees that it is their sole responsibility to ensure that their pet has all required documentation, vaccinations, treatments, testing, and certifications necessary for travel and re-entry into the United States.

    2. Acknowledgment of Resources

    The Client acknowledges that they have been informed of available resources, including but not limited to the USDA and CDC websites, to verify current travel requirements. The Client confirms they have been given the opportunity to review these resources and ask questions.

    3. Accuracy of Information Provided

    The Client certifies that all information provided to the Hospital, including but not limited to personal information, pet identification, medical and vaccination history, and all travel details (including destinations, dates, and transit plans), is true, complete, and accurate to the best of their knowledge. The Client understands that all documents prepared by the Hospital are based solely on the information provided by the Client. The Client agrees to promptly notify the Hospital of any changes to such information.

    4. Assumption of Risk

    The Client acknowledges that any inaccuracies, omissions, or changes in the information provided may result in delays, denial of entry, quarantine, additional costs, or other travel-related complications. The Client assumes full responsibility for any such outcomes.

    5. Limitation of Liability

    To the fullest extent permitted by law, the Client agrees that Animal Hospital of Fort Lauderdale, its veterinarians, employees, and agents shall not be held liable for any errors, omissions, delays, denied entry, quarantine, additional expenses, or other consequences arising from:

      •  Incomplete, inaccurate, or outdated information provided by the Client

      •  Changes in travel regulations or enforcement

      •  Missing or insufficient documentation

      •  Actions or decisions made by government agencies or foreign authorities

    6. Release and Hold Harmless

    The Client hereby releases, waives, and agrees to hold harmless Animal Hospital of Fort Lauderdale, its veterinarians, staff, and affiliates from any and all claims, liabilities, damages, losses, or expenses, including but not limited to loss, delay, denial of entry, or quarantine, arising out of or related to the Client's pet travel.

    7. Acknowledgment and Agreement

    By signing below, the Client acknowledges that they have read, understand, and agree to the terms of this Agreement, and that they accept full responsibility for ensuring their pet meets all travel requirements.

  • Note: Dogs that will be in the United States temporarily (such as for a vacation or other short-term travel) should have their temporary U.S. address listed. If the dog will be in the United States for less than one day and the owner does not have a U.S. address, including dogs whose owners live in Canada or Mexico, you may list the physical address of the U.S. veterinary clinic where the dog was vaccinated against rabies in this section. The address does not need to be updated on the form if it changes or for subsequent trips with a different temporary U.S. address.

  • Important Requirements — Rabies Vaccination

  • You are traveling on the same plane as your veterinarian must microchip the dog and administer a 1-year rabies vaccine.

    Key Requirements:
    1. The veterinarian must microchip the dog and administer a 1-year rabies vaccine.
    a. Rabies vaccinations administered prior to the microchip implantation date are invalid because they cannot be verified.
    2. Future rabies vaccinations may be valid for 3 years, but the initial vaccine administered after microchip implantation is only valid for 1 year.
    3. Do not enter any rabies vaccination information in the Veterinary Export Health Certification System (VEHCS) for vaccines administered prior to the microchip implantation date.
    4. The rabies vaccine must be given at least 28 days prior to issuance of this form (the Certification of U.S.-issued Rabies Vaccination form).
