Please read and complete this form to use the lake house amenities. Participation is optional; however, completion of this acknowledgment is required for access to these amenities.
Before enjoying the lake house amenities, please read and acknowledge each item below. This waiver is required for access to paddleboards, kayaks, hot tub, swimming, and other lake activities. Always use caution and follow posted rules.

Covered Activities
Risk Acknowledgement - I acknowledge that participation in lake activities, including kayaking, paddleboarding, swimming, and hot tub use, involves inherent risks, including injury, drowning, and property damage. I voluntarily choose to participate and accept these risks.
Personal Responsibility - I am responsible for my own safety at all times and agree to act with reasonable care while using the property and its amenities.
Safety Compliance - I agree to follow all posted rules and applicable local and state safety regulations, including proper use of life jackets where required. As part of our Pre-Arrival setup, we require a minimum of 72 hour notice for any specialty equipment required, including children life jackets and oversized jackets.
Supervision of Minors - I agree to supervise minors at all times and ensure that only capable individuals use watercraft.
Emergency Notification - I agree to notify the host if emergency services are contacted or if safety equipment (including fire extinguishers) is used.
Equipment Use & Condition - I acknowledge that kayaks, paddleboards, and water equipment are provided as a complimentary amenity. I accept responsibility for their use and agree to return all equipment in the same condition in which it was provided.
Damage & Loss Responsibility - I understand that I am financially responsible for any damage, loss, or misuse of equipment during my stay. I agree that reasonable repair or replacement costs may be requested through Airbnb/vrbo/security deposit if necessary.
Group Responsibility - I accept responsibility for all members of my group and any guests using the amenities during our stay.
Release of Liability - I release the owner and host from liability to the fullest extent permitted by law.
