Relate Counseling Center seeks to provide high-quality, low-cost outpatient mental health care to individuals, couples, and families experiencing financial hardship. It is Relate's goal to allow people access to therapy services, regardless of ability to pay. Please complete the information below to the best of your ability.

If you fall outside of our eligibility guidelines, please complete an application and provide additional information. We recognize that there may be extenuating circumstances impacting your financial situation, and we take these into consideration within the determination process.

 Please submit:

• Previous year's tax return. Include only the first 2 pages and black out your social security number, if possible.
• If no tax return is available, or your circumstances have changed, other acceptable forms of documentation are:
  • Most recent 3 paystubs
  • Most recent 3 bank statements
  • SSI/Disability letter
  • Food Stamp letter
  • Unemployment benefits letter

Once your application is reviewed, our Financial Assistance team will reach out to you to discuss next steps.
