  • In order for an application to be accepted; the submission must include the following:

     

    1) Proof of Residency via a driver's license photo showing a City of Josephine address; or a copy of a City of Josephine utility bill.  Employee's can utilize their work badge as proof of employment.

    2) Verification of military status via (DD214 or DOD1300); if still serving please visit the Department of Defense website and submit a copy of the service verification page of the service member.

    3) An official military photo of the person to be honored

    4) A completely filled out application
