Camp Wellness Enrollment
Thank you for your interest in participating in Camp Wellness. We are so glad that you are here! Enrollment Form Instructions: Please add participant information and contact below along with selecting age group (Elementary or Middle). Elementary School Groups occur on selected dates June-August, M-F 9:00am-12:00pm and Middle School Groups M-F 1:00pm-4:00pm. You may sign up for one week of Camp or multiple weeks. Early Bird Discount-Enrollment by May 1 for 10% discount. $100 off for enrollment in 2 weeks at a time (June, July, August). Camper is encouraged to attend all 5 days to get the most out of the experience. Upon enrollment, Parent/Guardian will be contacted to schedule a Welcome Session with more information via Google Meet. Following, a Camper Evaluation Session will be schedule to occur on Google Meet to include Parent/Guardian, Camper and Facilitators (Included in Camp fee).
