Waiver and Release Form
I understand that participating in dog training activities involves inherent risks to myself, members of my household, and my dog. By enrolling in classes or events with Do Over Dog Training, I voluntarily accept and assume all such risks.
I also understand that there are risks associated with being on the training property, including but not limited to uneven surfaces, equipment, dogs, and other conditions that could result in slips, trips, or falls. I accept these risks and agree to release and hold harmless Do Over Dog Training, its owner, instructors, employees, and representatives from any injury or accident that may occur on the property.
I agree to take full responsibility for my dog’s behavior and any damage or injury caused by my dog or by me. I will follow all instructions and safety guidelines provided by the instructor. I understand that failure to do so may result in dismissal from class without a refund.
Policies
- Tuition is non-refundable after the first class session.
- No regularly scheduled make-up classes are offered.
- If an instructor determines that a dog is not appropriate for a group class setting, remaining tuition may be applied toward private lessons.
- If classes cannot be held due to government-mandated closures, sessions may be conducted virtually. If I choose not to participate, I understand that tuition will be forfeited.
Health Requirements
I affirm that my dog is up to date on all appropriate vaccinations, free of communicable diseases, and free of parasites.
No Guarantee of Results
I understand that training results are not guaranteed and depend on many factors, including my dog and my own consistency and participation.
Media Release
I give permission to Do Over Dog Training and Lori Timberlake to use photos, video, or audio recordings of myself, my dog(s), and any minor children accompanying me for instructional, educational, and promotional purposes, including website, social media, and other materials. I release any claims related to the use of these materials.
Liability & Indemnification
I understand the risks associated with dog training, including the possibility of injury such as dog bites. I agree to release and hold harmless Do Over Dog Training and its representatives from any claims, damages, or expenses resulting from my participation or my dog’s behavior, including claims made by third parties.
Do Over Dog Training will make every reasonable effort to provide a safe training environment.
