By checking "yes" you give Willowdale Baptist Church camp staff/volunteers permission to take your child off church property to participate in camp activities.

RELEASE AND WAIVER OF LIABILITY
I, the undersigned, hereby declare and affirm that:

I am the parent/legal guardian of the youth named above (hereinafter referred to as "Child"), who is under my care and responsibility.

I hereby declare that my child is allowed to participate fully in activities associated with the WBC Summer Camp. I understand that at the event my child may participate in different activities that are supervised by staff and volunteers. I also agree that my child will be bound by the rules of the leaders of these activities and the rules of any place or organization where the activities may be held.

I understand that the program has activities that will involve physical contact with other participants, I have explained these risks and benefits of participating in this program to my child and my child is in proper physical condition and has no existing injuries or conditions that could jeopardize his/her safety or health, or the safety or health of the other participants.

I hereby release, waive, and discharge Willowdale Baptist Church including their staff, organizers, volunteers and participants, on my child’s behalf from any and all liability, loss, injury or death which may be suffered, sustained or arise in any way whatsoever as a result of or in conjunction with these activities. This event could involve the following possible injuries: Car accident, sun stroke, falls, body contact sports, and burns.

I also give permission for the staff, representative, or volunteers of Willowdale Baptist Church to administer first aid or to seek medical care for my child during my child’s participation in the program, including transportation of my child to a medical facility for additional treatment that appears necessary.
